We request that you arrive 10-15 minutes prior to your scheduled appointment for time to complete any paperwork. We strive to see all patients on time; however, there are times when our schedule is delayed in order to accomodate an emergency patient. Please accept our apology in advance should this occur during your appointment.
Please note that when you schedule an appointment, this time is reserved specifically for you. In the case that you are unable to make your appointment, we request that you provide our office with at least a 24 hour notice. If three (3) broken/missed appointments or three (3) cancellations occur without a 24-hour notice, our office reserves the right to not schedule any subsequent appointments. As friendly reminders, we will contact all patients by phone at least one (1) day prior to their appointments. Please ask about our new option of emailing for reminders!
The use of cellular phones is prohibited in the operatory. The extra conversation in the clinical area can be most distracting to others, preventing us from close, careful communication with each patient.
Please be aware that the PATIENT is legally responsible for payment of all charges, co-pays, deductibles, or other charges incurred. We gladly accept Cash, Check, and Credit Card payments. We cannot accept any post-dated checks. We do offer a third-party, payment plan through Care Credit; with zero-interest for twelve months for qualified customers.
There exists no direct relationship between our office and your insurance company. The type of plan chosen by you, and/or your employer determines your insurance benefits. As such, we have no say in the selection of your insurance company, no control over the contract terms, the reimbursement methods or the insurance benefits determination. We will accept assignment of benefits from your insurance company; however you are responsible for the full balance including any amount that is not paid by your insurance company.
The cost of the appliance must be paid on the day the impressions are taken. This is necessary because our office must pay the lab bills when appliances are ordered, not when they are completed.
All emergency treatment must be paid in full at the time the service is rendered.
